In order to use the City of Madison Employment Application you need to have the Plug-in for OmniForm Mailable Filler.

Open Internet Explorer and select the appropriate Employment Application (Web-based form created with Omniform software).

Look for a pale yellow alert bar at the top of the window that says "This site might require the following ActiveX Control: 'filename.ocx' from 'ScanSoft, Inc'. Click here to install..." The "filename.ocx" phrase will be the specific ActiveX Control file name you need to download in order to use the form. When you see this, click the bar.

Click the "OK" button when prompted after the ActiveX Control installs.
